The Community Living Program (CLP) provides consultation, assessment, and intervention services to assist Howard County residents, 18 years or older with a disability, remain living at home in the community.

The person-centered approach focuses on ensuring safety, promoting independence and maintaining quality of life. An Occupational Therapy practitioner may conduct a comprehensive in-home or virtual assessment to evaluate the environment for safety risks and decreased function.

The therapist works with individuals to prioritize areas of need and appropriate solutions. Frequent interventions involve:

  • Development of emergency response plans
  • Removal of environmental hazards
  • Training on adaptive equipment
  • Education in adaptive or compensatory strategies during daily activities
  • Recommendations may be made for durable medical equipment and/or adaptive equipment to improve safety and independence in the home

Services are determined and provided based on household income and priority is given to lower income status. All services are subject to the availability of funds.
