About
Youth, Young Adult, & Family Behavioral Health Care Coordination
Youth and young adult care coordination connects families dealing with behavioral health challenges to resources and referrals in the community that meet their needs.

Care Coordination services empower families to achieve positive outcomes through a structured, family-centered process. Children between the ages of 4 and 18 who are in the public mental health system may access care coordination services. For more information, use this referral form:

If a family member or client under 18 years old would benefit from residential treatment and you would like to make a referral, contact Program Coordinator Jocelyn Tillman at jtillman@howardcountymd.gov.
The Howard County Local Care Team (LCT) is an interagency council designed to provide resources and support to families who need to access services for a child with intensive emotional and behavioral needs.
