Vaccinations are FREE and available for all Marylanders age 5 and older. Anyone who is part of an eligible age group can register for a vaccine using the links below or at many pharmacies or medical providers.

Health Department Vaccination Clinics
The Health Department offers first shots, second shots and booster shots to all eligible individuals at open clinics - verify vaccine type and age eligibility before making an appointment. Important Information:
- If you are unable to complete the online registration or have additional questions about the COVID-19 vaccine, call 410-313-6284. Operators are standing by Monday - Friday from 8:30am - 4:30pm.
- The Health Department only stocks the Pfizer vaccine for children and adolescents ages 6 - 17. Please note the vaccine type and age range for a designated clinic before making your appointment. For infant/toddler vaccination, click here.
- Booster shots are authorized for individuals ages 5 and older.
- Minor consent forms for COVID-19 vaccination can be found here: Pfizer Vaccine Minor Consent Forms. (Signed consent form is required if parent/guardian is unable to attend either vaccine dose appointment)
- Individuals in need of a second vaccine dose can schedule an appointment using the links below, call 410-313-6284 or email dose2@howardcountymd.gov.
- Anyone receiving a third/booster vaccine shot should bring their vaccine card or other proof of vaccination to verify the date of vaccination.
- Azova Saliva PCR tests are offered at many Health Department COVID-19 vaccine clinics. One take home test kit per person, per appointment. Testing kits are not available at clinics hosted by other organizations. Find more information about Azova saliva PCR testing and make your appointment here.

Pfizer Vaccine Minor Consent Forms
Parents of minor children should complete a vaccine consent form and bring it with them for their child's vaccination appointment.
The State of Maryland has a resource that allows you to search for a vaccination clinic near you. Go to the following: coronavirus.maryland.gov/pages/vaccine.
The Maryland Department of Health launched a COVID-19 Vaccination Support Center to provide Marylanders without technology capabilities the ability to gain access to COVID-19 vaccine information and appointments. Call 1-855-MDGOVAX (1-855-634-6829) 7AM - 10PM 7 days a week.

Locating Your Official Vaccination Records
Maryland MyIR is a free website service that allows consumers to view and print copies of their official vaccination records directly from ImmuNet, Maryland's immunization information system. ImmuNet is used by healthcare providers to securely maintain vaccination records for Maryland residents of all ages.

After completing your on-site registration for your second vaccine dose, you will receive a confirmation email from "Vaccination Clinics " with the details of your second dose appointment. You will receive a reminder email 24 hours before your second dose appointment from the same email address. If you have received this message, your second dose is confirmed and you do not need to do anything else. If you have not received the confirmation message, please check your spam folder. If you cannot locate your second dose appointment confirmation, you can call us at 410-313-6284 or email dose2@howardcountymd.gov.
